Implication

Use AST ≥140 bpm to enhance CSRTI reproducibility in sinus node evaluation; leaves open optimal timing and rates for diagnostic yield.

Structured PICO

Does the time of day and atrial overdrive stimulation rate affect the reproducibility and diagnostic yield of corrected sinus node recovery time in patients being evaluated for sick sinus syndrome?

P
Population
15 patients (9 group I patients with 'prolonged' CSRT max = 3,196 + 2,740 msec and 6 group II patients with 'short' CSRT max = 367 + 79 msec)
I
Intervention
Atrial overdrive stimulation rates (AST) of 90, 110, 140, 170, and 200 bpm at 0800, 1100, 1400, 1700, 2000, and 2300 hours on three consecutive days using a loop-mounted stable atrial electrode
O
Outcome
Corrected sinus node recovery time (CSRT) values and their reproducibilitysurrogate

The optimal diagnostic yield for sick sinus syndrome testing using corrected sinus node recovery time is significantly influenced by the time of day and the atrial stimulation rate used.
