Population

Approximately 12,000 insurants aged ≥18 years of the KKH statutory health insurance in Germany, with one or more of the following chronic conditions: diabetes, coronary artery disease, asthma, hypertension, heart failure, COPD, chronic depression, or schizophrenia. For specific conditions (type 2 diabetes, hypertension, coronary artery disease), a risk score for hospital readmission within the next year must be >50%.

Intervention

Telephone-based health coaching (TBHC) carried out by trained nurses, tailored to patients' needs and medical condition, following motivational interviewing and shared decision-making. Minimum frequency of one telephone contact every six weeks, with a maximum duration of two years.

Comparator

Usual care (no coaching).

Outcome

Time until hospital readmission within two years after enrolling in the health coaching, assessed by routine data.hard clinical
