Four cases of pulmonary sporotrichosis have been observed in three institutions in Ohio. Three of these cases represent primary pulmonary infection, and one represents disseminated disease associated with sarcoidosis. The occupational hazard of gardening or handling vegetable produce was noted in three of the four patients. Cavitary lesions observed in three of the patients were treated by pulmonary resection and chemotherapy with amphotericin B and iodides. Three patients were well 10 to 52 months after treatment, but active pulmonary sporotrichosis was found in the fourth case apparently associated with active pulmonary tuberculosis. A possible relation between infection with Sporotrichum schenckii and sarcoidosis or prolonged steroid therapy (or both) in one case is proposed.
