Why the study?

Cortex-involved lesions are typically associated with embolic stroke commonly caused by AF, but the associations between cortex-involved stroke, vascular risk factors, and AF subtypes remained to be determined.

Is newly detected atrial fibrillation associated with cortex-involved ischemic stroke in acute ischemic stroke patients?

Population

243 acute ischemic stroke patients with AF at 20 Chinese hospitals

Comparison

Newly detected AF vs known AF across AF duration subtypes

Design

Prospective, multicenter cohort, cross-sectional imaging sub-study with blinded imaging review

Implication

Newly detected AF was associated with cortex-involved stroke; leaves open whether AF subtypes guide targeted screening or secondary prevention.

Structured PICO

Is newly detected atrial fibrillation associated with cortex-involved ischemic stroke in acute ischemic stroke patients?

P
Population
243 acute ischemic stroke patients with atrial fibrillation (within 7 days after stroke onset) with required MRI sequences, from a prospective multicenter cohort across 20 Chinese hospitals.
I
Intervention
Newly detected atrial fibrillation (diagnosed by 6-day Holter monitoring and classified by duration >24 h or ≤24 h)
C
Comparator
Previously known atrial fibrillation
O
Outcome
Cortex-involved ischemic stroke lesions on diffusion-weighted MRIsurrogate

Newly detected atrial fibrillation persistent for more than 24 hours is significantly associated with cortex-involved ischemic stroke, suggesting a cardioembolic mechanism for the index event.

Limitations

  • Exact etiologic classification of stroke or TIA could not be obtained due to lack of imaging information on large artery diseases.
  • Paroxysmal AF or pre-existing AF might be included in patients with newly detected AF on account of insufficient screening before stroke.
  • Small sample size restricted subdividing the duration of AF into shorter or longer hours.
