Overview

Right-sided ISB was associated with HRV shifts suggesting autonomic effects; leaves open clinical relevance and need for randomized confirmation.

Structured PICO

Does right-sided versus left-sided interscalene brachial plexus block differentially affect heart rate variability in patients undergoing shoulder surgery?

P
Population
24 adult patients scheduled for elective arthroscopic or open shoulder surgery in sitting position (n=14 right-sided, n=10 left-sided), mean age 38.2 (Group R) and 32.1 (Group L), predominantly male. Key exclusions: BMI > 35, coronary artery disease, arrhythmia, congestive heart failure, diabetes mellitus, autonomic dysfunction.
I
Intervention
Right-sided interscalene brachial plexus block (ISB) using 40 ml of 0.5% ropivacaine without epinephrine.
C
Comparator
Left-sided interscalene brachial plexus block (ISB) using 40 ml of 0.5% ropivacaine without epinephrine.
O
Outcome
Changes in heart rate variability (HRV) parameters (specifically natural logarithm of low frequency [InLF]) at 30 minutes post-block.surrogate

Right-sided, but not left-sided, interscalene brachial plexus block alters cardiac autonomic outflow by reducing sympathetic and increasing parasympathetic tone.
