Pulsed field ablation shows encouraging early results versus thermal energy for VT in NICM.
Why the study?
Ventricular tachycardia ablation in nonischemic cardiomyopathy remains limited by high recurrence rates due to heterogeneous, intramural, or epicardial substrates that make durable lesion formation with thermal energy difficult.
Does pulsed field ablation improve outcomes compared to thermal ablation in patients with nonischemic cardiomyopathy and ventricular tachycardia?
Does pulsed field ablation improve outcomes compared to thermal ablation in patients with nonischemic cardiomyopathy and ventricular tachycardia?
Pulsed field ablation is emerging as a promising alternative to conventional thermal ablation for addressing the challenging arrhythmogenic substrates in nonischemic cardiomyopathy.
PFA remains investigational for VT ablation in NICM; leaves open whether it improves outcomes over thermal energy.
Ventricular tachycardia ablation in nonischemic cardiomyopathy remains limited by high recurrence rates despite advances in catheter-based techniques. In nonischemic cardiomyopathy, the arrhythmogenic substrate is often heterogeneous and may involve intramural or epicardial myocardium, making durable lesion formation with conventional thermal energy difficult. Pulsed field ablation uses irreversible electroporation rather than thermal injury and is being investigated as an alternative approach for these challenging substrates. Early preclinical and clinical experience has shown encouraging results, with no major safety concerns identified so far. This review examines the current challenges of thermal ventricular tachycardia ablation, summarizes the current evidence for pulsed field ablation in this group of patients, and highlights knowledge gaps and future directions.

Kamel et al. (2026) conducted a review in Ventricular tachycardia in nonischemic cardiomyopathy. Pulsed field ablation vs. Conventional thermal energy was evaluated. Pulsed field ablation is being investigated as an alternative to conventional thermal energy for ventricular tachycardia ablation in nonischemic cardiomyopathy, with early experience showing encouraging results.
