EMR-integrated multidisciplinary delivery plan enables uncomplicated vaginal delivery in unrepaired atrioventricular septal defect.
Why the study?
Pregnant patients with congenital heart disease, particularly unrepaired defects, require specialized multidisciplinary care to optimize maternal and fetal outcomes.
Case Report (n=1)
Embedding a multidisciplinary, individualized delivery plan into the electronic medical record ensures consistent communication and can facilitate safe peripartum management in pregnant patients with complex congenital heart disease.
May support EMR-embedded multidisciplinary plans for peripartum care in unrepaired AVSD; hypothesis-generating pending larger studies.
BACKGROUND: Pregnant patients with congenital heart disease (CHD), particularly those with unrepaired defects, require specialized multidisciplinary care to optimize maternal and fetal outcomes. CASE SUMMARY: A 38-year-old gravida 5 para 2 woman presented at 20 weeks of gestation with palpitations and a murmur. Evaluation revealed a transitional atrioventricular septal defect with moderate left atrioventricular valve regurgitation. Medical management was guided by multidisciplinary review, and a detailed delivery plan was embedded in the electronic medical record (EMR). She developed pre-eclampsia at 37 weeks and underwent vaginal delivery. DISCUSSION: Patients with unrepaired atrioventricular septal defect are at increased risk for pregnancy complications and postpartum decompensation. EMR-integrated delivery plans improve team coordination and reduce risk by ensuring consistent communication across specialties. This case highlights the practical application of such tools in CHD pregnancy management. TAKE-HOME MESSAGE: Multidisciplinary collaboration and EMR-integrated delivery plans are critical to improving peripartum outcomes in pregnant patients with CHD.

Bartra et al. (2025) conducted a case report in Unrepaired congenital heart disease in pregnancy (n=1). Multidisciplinary care with an EMR-integrated delivery plan was evaluated on Maternal and fetal clinical course. An EMR-integrated multidisciplinary delivery plan successfully guided the management and uncomplicated vaginal delivery of a pregnant patient with an unrepaired atrioventricular septal defect.
