Key result
Spontaneous premature atrial complexes occurring at His refractoriness demonstrated the electrophysiological mechanisms of atrioventricular nodal reentrant tachycardia, excluding junctional tachycardia.
Why the study?
His refractory atrial extrastimulus can help in the differential diagnosis between atrioventricular nodal reentrant tachycardia and junctional tachycardia.
Can spontaneous premature atrial contractions during His refractoriness facilitate the differential diagnosis between atrioventricular nodal reentrant tachycardia and junctional tachycardia?

Can spontaneous premature atrial contractions during His refractoriness facilitate the differential diagnosis between atrioventricular nodal reentrant tachycardia and junctional tachycardia?
Spontaneous premature atrial contractions occurring during His refractoriness can serve as a useful diagnostic tool to differentiate AVNRT from junctional tachycardia.
May aid EP differentiation of AVNRT from junctional tachycardia; leaves open broader adoption pending prospective validation.
His refractory atrial extrastimulus can help for differential diagnosis between atrioventricular nodal reentrant tachycardia and junctional tachycardia. In the present patient, spontaneous premature atrial complexes occurred at junctional depolarization, i.e. His refractoriness. Furthermore, the premature atrial complexes did not influence the immediate beat with no change on the following H-H interval. These observations demonstrated the electrophysiological mechanisms of atrioventricular nodal reentrant tachycardia, with the exclusion of junctional tachycardia.

Guan et al. (2021) conducted a case report in Atrioventricular nodal reentrant tachycardia (n=1). Spontaneous premature atrial contractions was evaluated on Differential diagnosis between atrioventricular nodal reentrant tachycardia and junctional tachycardia. Spontaneous premature atrial complexes occurring at His refractoriness demonstrated the electrophysiological mechanisms of atrioventricular nodal reentrant tachycardia, excluding junctional tachycardia.
