Does an 8-week aerobic exercise programme improve biomarkers of endothelial function and inflammation in patients after acute myocardial infarction?
p-value: p=<.05
An 8-week aerobic exercise program in post-MI patients increases anti-inflammatory IL-10 levels and prevents deterioration of endothelial function.
Supports aerobic exercise in post-MI rehabilitation; extends RCT evidence for its role in secondary prevention.
Purpose to assess the effects of exercise training on biomarkers of endothelial function and inflammation. Methods 47 patients after acute myocardial infarction were randomized to an exercise‐training group (ETG) receiving an 8‐week aerobic exercise programme (3× per week) or to a control group (CG) receiving usual care. At baseline and after 8 weeks the circulating levels of soluble intercellular adhesion molecule‐1 (sICAM‐1) and vascular cell adhesion molecule‐1 (sVCAM‐1), C‐reactive protein (CRP), interleukin (IL)‐6 and ‐10 were assessed. Results ETG increased the circulating levels of IL‐10. Conversely, CG did not change the inflammatory biomarkers, but increased the endothelial dysfunction (). Changes in the inflammatory and endothelial function biomarkers [median (percentile 25 th –75 th )] ETG CG Baseline Final Baseline Final CRP (mg/L) 1.1 (0.5–2.2) 1.4 (0.5–2.4) 0.9 (0.5–1.7) 0.9 (0.7–1.9) IL‐6 (pg/mL) 2.5 (1.6–3.2) 3.13 (2.1–5.0) 3.6 (1.7–6.5) 3.5 (2.8–4.7) IL‐10 (pg/mL) 5.8 (2.6–9.1) 6.36 (4.1–14.7) 7.7 (4.4–17.2) 7.3 (2.9–10.2) sICAM‐1 (ng/mL) 169 (143–209) 187 (154–201) 180 (147–219) 206 (169–242) sVCAM‐1 (ng/mL) 297 (223–377) 340 (269–418) 245 (179–304) 341 (231–424) Different from baseline, P<.05 Conclusions Exercise‐training revealed an anti‐inflammatory effect, expressed by the IL‐10 increase, and counteracted the endothelial function deterioration.

Ribeiro et al. (2010) conducted an RCT in acute myocardial infarction (n=47). Aerobic exercise programme vs. Usual care was evaluated on Biomarkers of endothelial function and inflammation (sICAM-1, sVCAM-1, CRP, IL-6, IL-10) (p=<.05). An 8-week aerobic exercise program increased anti-inflammatory IL-10 levels and counteracted endothelial function deterioration compared to usual care in patients after acute myocardial infarction.
