Implication

May explain low HDL-C in hypertriglyceridemia; hypothesis-generating and requires validation in larger studies.

Structured PICO

Does triglyceride enrichment of HDL enhance the metabolic clearance of HDL apo A-I in healthy men?

P
Population
12 healthy, normolipidemic men (n=6 in kinetic study, n=6 in composition study), mean age 29.7 +/- 2.7 years (kinetic group), mean BMI 24.4 +/- 1.2 kg/m2. Nonsmokers, no history of cardiovascular disease or systemic illness, not taking any pharmacologic agents.
I
Intervention
Intravenous infusion of synthetic triglyceride emulsion (Intralipid) for 5 hours (50 cc/h) to generate TG-enriched HDL, which was then radiolabeled and injected intravenously.
C
Comparator
Autologous fasting HDL (isolated prior to Intralipid infusion), radiolabeled and injected simultaneously.
O
Outcome
Fractional catabolic rate (FCR) of HDL-associated apo A-I over a 5-day periodsurrogate

Triglyceride enrichment of HDL directly enhances the metabolic clearance of HDL apo A-I in humans, providing a mechanistic explanation for low HDL cholesterol in hypertriglyceridemic states.
