Phantom limb pain (PLP) is a chronic neuropathic pain condition frequently refractory to pharmacological therapy alone. Mirror therapy is a simple, noninvasive intervention targeting maladaptive cortical reorganization. A 34-year-old male with chronic PLP following left above-elbow amputation presented with severe burning pain and allodynia inadequately controlled with medications. Mirror therapy was introduced as an adjuvant to optimized treatment with pregabalin and duloxetine. A significant reduction in pain intensity was observed within 4 weeks. Mirror therapy may be a safe, feasible, and effective adjuvant to pharmacotherapy in patients with chronic PLP.
