Key result
Magnesium orotate added to amiodarone therapy increased sinus rhythm restoration to 67.8% compared to 57.2% with amiodarone alone and significantly reduced supraventricular extrasystoles in patients with paroxysmal atrial fibrillation.
Why the study?
Does the addition of magnesium orotate to amiodarone improve the restoration of sinus rhythm, reduce electrical instability, and improve quality of life in patients with paroxysmal atrial fibrillation?

Does the addition of magnesium orotate to amiodarone improve the restoration of sinus rhythm, reduce electrical instability, and improve quality of life in patients with paroxysmal atrial fibrillation?
Absolute Event Rate: 67.8% vs 57.2%
The addition of magnesium orotate to amiodarone for medical cardioversion in patients with paroxysmal atrial fibrillation reduces subsequent atrial electrical instability and enhances quality of life.
Should not yet change amiodarone monotherapy in paroxysmal AF; leaves open adjunctive magnesium orotate benefit.
56 patients (16 women and 40 men) with average age of 61.5 ±4.3 years, having paroxysm of atrial fibrillation were examined. All patients were divided into two groups. Patients of the 1 st group (n=28) underwent medical cardioversion with amiodarone and magnesium orotate was prescribed. Patients of the 2nd group (n=28) received only amiodarone. They estimated time before the restoration of sinus rhythm in both groups, electrical instability of the atrial myocardium and ventricles after restoration of sinus rhythm, patients’ quality of life in 3 months after release from the hospital. It was noted that the addition to the basic therapy of patients with atrial fibrillation of magnesium orotate increases the effectiveness of drug cardioversion and shortens the time of its onset. When using amiodarone and magnesium orotate, signs of electrical instability of the atrial myocardium are significantly less often detected using Holter ECG monitoring (number of paired and supraventricular extrasystoles) compared with amiodarone monotherapy. Atrial fibrillation caused worsening of the quality of life in the form of increased severity of symptoms of the disease, limited physical activity, troubles in the psycho-emotional sphere. After 3 months of treatment it was noted that QL in complex therapy with magnesium orotate in patients of the 1st group was 15.7% higher compared with patients of the 2nd group.

Фуштей et al. (2017) conducted an RCT in Paroxysmal atrial fibrillation (n=56). Magnesium orotate and amiodarone vs. Amiodarone alone was evaluated on Restoration of sinus rhythm. Magnesium orotate added to amiodarone therapy increased sinus rhythm restoration to 67.8% compared to 57.2% with amiodarone alone and significantly reduced supraventricular extrasystoles in patients with paroxysmal atrial fibrillation.
