To determine whether cruciform soft-tissue reconstruction after patellectomy successfully restores extension torque and centralizes the extensor mechanism.
Evaluated 8 patients (12 patellectomies) undergoing four-quadrant cruciform pants-over-vest extensor mechanism reconstruction incorporating the vastus medialis insertion.
Assessed knee extension and flexion function along with extensor-to-flexor torque ratios using bilateral Cybex dynamometry at a mean follow-up of 56 months (range, 18 months to 20 years).
Subjective knee evaluations remained high with few patient complaints, demonstrating a mean knee score of 94 (median, 100).
Objective Cybex dynamometry showed incomplete restoration of extensor torque and a moderate deficit in quadriceps function.
The ratio of extension to flexion forces differed significantly between patellectomized knees and intact contralateral knees in unilateral cases, with similar abnormal ratios observed in bilateral patellectomies.
