Myocardial bridging of the LAD can cause subendocardial infarction, simulate coronary spasm on angiography, and is amenable to surgical correction.
Myocardial bridging may cause ischemia amenable to resection; single case leaves open its prevalence and management indications.
• A case of subendocardial injury was secondary to myocardial bridging of the left anterior descending coronary artery. Coronary arteriography showed a marked systolic narrowing at the midportion of the left anterior descending coronary artery consistent with myocardial bridging. The constricting band was confirmed and resected at surgery. It is concluded that myocardial bridging (1) is not a totally benign condition, (2) is amenable to surgical correction in selected cases, and (3) can simulate coronary arterial spasm during selective cineangiography.
