To assess how the rising use of immunosuppressants over a 25-year period influenced the need for intestinal surgery and complications in Crohn's disease.
Retrospective medical chart review of 2573 patients diagnosed with Crohn's disease across five consecutive cohorts (1978–1982, 1983–1987, 1988–1992, 1993–1997, and 1998–2002).
Assessed the initiation of immunosuppressants (azathioprine or methotrexate), need for intestinal resection, and intestinal complications using Kaplan-Meier survival analysis.
Stratified the primary analysis into 565 early-referral patients (evaluated within 3 months of diagnosis) and 2008 late-referral patients.
The 5-year cumulative probability of receiving immunosuppressive therapy increased significantly from 0 in the 1978–1982 cohort to 0.13, 0.25, 0.25, and 0.56 in subsequent cohorts through 1998–2002 (p < 0.001).
The 5-year cumulative risk of intestinal resection remained unchanged over time, shifting from 0.35 in the earliest cohort to 0.34 in the most recent cohort (p = 0.81).
Rates of stricturing and penetrating intestinal complications remained stable across all time cohorts, with consistent outcomes observed in the late-referral group of 2008 patients.
