To describe the surgical management of a solid pseudopapillary neoplasm using laparoscopic spleen-preserving distal pancreatectomy in a patient with an anomalous splenic vein confluence.
Evaluated an asymptomatic 37-year-old male presenting with a solid pseudopapillary neoplasm using preoperative computed tomography angiography to map peripancreatic vascular anatomy.
Performed a laparoscopic spleen-preserving distal pancreatectomy using a superior pancreatic approach to isolate and preserve the anomalous splenic vein and splenic artery.
Computed tomography angiography revealed an anomalous splenic vein traveling transversely along the superior border of the pancreas over the celiac artery into the portal vein.
Laparoscopic resection was executed successfully without peripancreatic vessel injury, achieving complete tumor removal while preserving splenic perfusion.
