Review outlines glycocalyx degradation and therapeutic restoration across aging and chronic conditions, highlighting its fundamental role as an active regulatory meta-medium.
Glycocalyx has long been thought to be a passive mucus layer, but modern research has redefined this notion. It is a complex, dynamic, nano-sized sugar coating that covers all human cells and functions as an active medium for cellular identity, mechanotransduction, immune surveillance, and systemic regulation. The thickness of glycocalyx reaches 500 nm, its area on the endothelium is thousands of square meters - this is the largest meta-environment of the body in terms of area. The article substantiates that glycocalyx is a meta-medium No20 that meets all four criteria: ubiquity, functional integrity, communicative nature, intrinsic pathology. With age, glycocalyx thins, loses sialic acids, mucin-type O-glycosylation is impaired, which leads to increased vascular permeability, neuroinflammation, cognitive impairment and systemic inflammation. Dysregulation of glycocalyx is a key link in the pathogenesis of atherosclerosis, sepsis, diabetes, Alzheimer's disease, and cancer. Of particular significance is the 2025 discovery showing that the restoration of mucin-type O-glycans improves the function of the blood-brain barrier and reduces cognitive impairment in old mice. Systemic restoration of glycocalyx in the Quatotron method through multimodal physical action is presented.
