Why the study?
Solid evidence for treating elderly PAH patients with cardiopulmonary comorbidities remains lacking, complicating management.
Highlights the complexity and lack of solid evidence in managing elderly PAH patients with cardiopulmonary comorbidities.
Supports close monitoring for left-heart unmasking after initial combination; leaves open first-line therapy in elderly PAH with cardiopulmonary comorbidities.
Elderly patients with pulmonary arterial hypertension (PAH) exhibit distinct features and outcomes compared to classical patients. Cardiopulmonary comorbidities are common and likely contribute to the development of pulmonary hypertension (PH). However, solid evidence for treating elderly PAH patients with these comorbidities remains lacking, complicating management. We report a case of a 74-year-old male with PAH and cardiopulmonary comorbidities who responded transiently to initial combination therapy with pulmonary vasodilators.
