Why the study?
Does the site of obstruction and collateral circulation affect clinical presentation and prognosis in patients with isolated left anterior descending coronary artery disease?
Population
218 patients with significant obstruction confined to the left anterior descending coronary artery
Comparison
Proximal LAD lesions or absence of collateral… vs Distal LAD lesions or presence of collateral…

Proximal LAD lesions were associated with worse LV function and prognosis; leaves open whether lesion-specific strategies improve outcomes.
Does the site of obstruction and collateral circulation affect clinical presentation and prognosis in patients with isolated left anterior descending coronary artery disease?
In patients with isolated LAD disease, proximal lesions and lack of collaterals are associated with worse left ventricular function and poorer prognosis, suggesting that expanding CABG indications beyond severe angina is unlikely to improve survival in those with already poor LV function.
