Robotic bronchial sleeve resection with 3D planning achieves negative margins while preserving ipsilateral lung parenchyma.
Why the study?
Robotic left main bronchial sleeve resection for endobronchial tumors can avoid pneumonectomy while preserving lung parenchyma.

Robotic left main bronchial sleeve resection with preoperative 3D planning and intraoperative fluorescence imaging is a feasible lung-sparing approach for endobronchial carcinoid tumors.
Robotic sleeve resection may enable lung preservation in select left main tumors; leaves open safety and efficacy in larger cohorts.
Robotic left main bronchial sleeve resection for endobronchial tumors avoids pneumonectomy while preserving the entire ipsilateral lung parenchyma. Preoperative three-dimensional computed tomography reconstruction guides dissection around the pulmonary artery, pulmonary veins, and left atrium, and Firefly fluorescence imaging enables precise intraoperative tumor localization without needle puncture. We hereby describe a patient with a typical carcinoid tumor of the left main bronchus, who successfully underwent a minimally invasive lung -preserving operation to achieve negative surgical margins via a left transthoracic approach.

Downes et al. (2026) conducted a case report in Typical carcinoid tumor of the left main bronchus (n=1). Robotic left transthoracic left main bronchial sleeve resection with preoperative 3D planning was evaluated on Surgical margins and lung preservation. Robotic left transthoracic left main bronchial sleeve resection with preoperative 3D planning successfully achieved negative surgical margins while preserving the ipsilateral lung parenchyma.
