To evaluate the physiological status of high-risk surgical patients and assess how well standard diagnosis-related group (DRG) classifications reflect their clinical severity and resource utilization.
Prospective study of 100 consecutive high-risk elective surgical patients admitted to a preoperative intensive care unit at a university tertiary referral center.
Hemodynamic status and physiologic staging were measured using Swan-Ganz catheters placed 1 to 2 days prior to operation to guide preoperative clinical management.
Physiologic staging categorized 0% of patients into stage 1, 55% into stage 2, 41% into stage 3, and 4% into stage 4, with stage 4 accounting for 3 of the 4 total deaths (4% overall mortality, all involving advanced liver disease).

Preoperative monitoring guided fine-tuning interventions in 53% of patients, and average total hospital length of stay reached 24.5 days despite DRG schemes providing only 15% supplemental reimbursement for complex comorbidities.
