The transjugular approach for percutaneous mitral commissurotomy is a feasible and advantageous alternative in patients with difficult anatomy, such as inferior vena cava obstruction.
May enable mitral commissurotomy when femoral access is precluded; hypothesis-generating and requires validation before wider use.
Percutaneous transvenous mitral commissurotomy was performed successfully via the transjugular approach in a patient with severe rheumatic mitral stenosis and obstruction of the inferior vena cava due to prior liver transplantation. This case demonstrates the advantage of the jugular approach in patients with difficult anatomy. Cathet Cardiovasc Intervent 2003;59:261–265.
