SUMMARY One hundred and sixty‐five cases of retropubic prostatectomy are presented. A simple technique is outlined and its short and long term results evaluated. Spinal anæsthesia augments wound relaxation during surgery and frequently avoids chest complications in the older age group. Simultaneous elective hernioplasty at the time of prostatectomy is strongly advocated. Routine vasectomies prevent epididymitis. Sexual function is preserved at the pre‐operative level. The high incidence of late vesical neck obstruction following retropubic prostatectomy is noted. The development of muscular hypertrophy is most interesting and its development is unexplained.
