Clinical trials suggest that elderly patients with stable angina and multivessel disease benefit more from revascularization than younger patients, though treatment selection requires a heart team.
Why the study?
Does revascularization with optimal medical therapy improve outcomes compared to optimal medical therapy alone in elderly patients with stable angina?
Does revascularization with optimal medical therapy improve outcomes compared to optimal medical therapy alone in elderly patients with stable angina?
Age alone should not preclude revascularization in elderly patients with stable angina, as they may derive greater benefit than younger patients, necessitating individualized heart team decisions.
May support heart team revascularization decisions in elderly stable angina; leaves open confirmation versus optimal medical therapy in dedicated trials.
Multivessel coronary disease is more common in elderly patients. The question is which primary strategy should be chosen for elderly patients with stable angina, i.e., revascularization (percutaneous coronary intervention or coronary artery bypass surgery) with intensive pharmacological therapy and lifestyle changes (optimal medical therapy) or optimal medical therapy alone. Clinical trials that compare these two approaches showed that elderly patients benefit more from revascularization than younger patients. There is no doubt that multivessel disease requires a heart team to select the optimal strategy. Age itself is not a criterion for selecting optimal treatment strategies. One must consider many other factors, including medical findings, angiography, the patients’ wishes, social and cultural aspects, and other parameters. Patients should be given complete and objective information.

Bogachev et al. (2012) conducted a review in Stable angina with multivessel coronary disease. Coronary revascularization (PCI or CABG) with optimal medical therapy vs. Optimal medical therapy alone was evaluated. Clinical trials suggest that elderly patients with stable angina and multivessel disease benefit more from revascularization than younger patients, though treatment selection requires a heart team.
