症例1（母）は48歳女性．Marfan症候群および胸部大動脈拡張でフォローされていた．胸部痛を主訴に搬送され，造影CTで急性大動脈解離（Stanford A型）と診断した．大動脈弁閉鎖不全症および心嚢水の貯留も認め，緊急でBentall手術を施行した．症例2（娘）は妊娠39週の26歳女性で，Marfan症候群の診断基準は満たしていなかった．母親が大動脈解離発症で救急を受診した際に背部痛が出現した．胎児への影響を懸念して患者が造影剤の使用を拒否したため，緊急で帝王切開を施行した．その後，造影CTで急性大動脈解離（Stanford B型）と診断した．そして，降圧療法を開始した．両症例共に軽快に退院した．症例2はMarfan症候群の基準は満たしていないものの，遺伝的素因があるなど，Marfan症候群の疑いが濃厚な妊娠例であった．このような症例については帝王切開も視野に入れて，密な周産期管理を行う必要があると考えられる．
