Collaborative working by cardiologists and specialist palliative care services brought about positive changes and allowed for earlier discussion of preferred place of care for a patient with chronic heart failure.
Case Report (n=1)
Early referral and collaborative care between cardiology and palliative services provide beneficial changes and facilitate end-of-life planning for heart failure patients.
Supports integrated HF palliative models; hypothesis-generating and should not yet change practice.
This short case study will illustrate how collaborative working by cardiologists and specialist palliative care services can bring about positive and beneficial changes for those patients suffering from chronic heart failure. It will also demonstrate that earlier referral to palliative care will ensure that matters such as preferred place of care, can be discussed and put into place for the appropriate time. The case study will reveal that patients and their carers accept referral to palliative care services and see the benefit of earlier referral. It is further acknowledged that all referrals to specialist palliative care are a learning experience for both disciplines.
