Why the study?
Parturients with acquired and congenital heart diseases experience puerperal cardiovascular and pulmonary changes at a greater magnitude, imposing multiple perioperative challenges for clinicians.
Highlights the importance of multidisciplinary perioperative collaboration and tailored anesthetic management in parturients with complex congenital and acquired heart disease.
Case report of complex cardiac twin pregnancy highlights management challenges; leaves open optimal protocols pending larger studies.
A BSTRACT Physiologic changes in the puerperium cause a major burden in the cardiovascular and pulmonary system; furthermore, parturients with acquired and congenital heart diseases and their sequelae experience these changes at a greater magnitude, imposing multiple perioperative challenges for clinicians. This paper reports a 20-year-old parturient with multivalvular heart disease and a patent ductus arteriosus with severe pulmonary artery hypertension with a discordant twin pregnancy. In this paper, we highlight the importance of a multidisciplinary perioperative collaboration and tailored anesthetic management when confronted with such a challenging case.
