To compare the efficacy and safety of bolus propofol versus ketamine as an adjuvant to remifentanil-based total intravenous anesthesia in pediatric rigid bronchoscopy.
Randomized 40 pediatric patients under 12 years of age scheduled for rigid bronchoscopy to receive either propofol (Group P) or ketamine (Group K).
Administered midazolam premedication, mivacurium for muscle relaxation, and a continuous remifentanil infusion (1 µg/kg/min) supplemented with bolus doses of the assigned adjuvant drug.
Maintained remifentanil at 0.05 µg/kg/min until extubation while monitoring hemodynamics, recovery characteristics, and adverse complications.
Mean arterial pressure decrease from baseline to the lowest intraoperative value was significantly greater in Group P than in Group K (p = 0.049).
Need for assisted or controlled mask ventilation following extubation occurred more frequently in Group K than in Group P.
Reductions in other hemodynamic variables and overall adverse event frequencies did not differ significantly between groups.
