Occult obstructive ureteropathy complicating Crohn's disease is reported in six patients. Primary intestinal resection and ureterolysis have been successfully employed in all six patients to achieve uncomplicated urinary tract decompression without morbidity or mortality. This constitutes the first series in which direct operative decompression by ureterolysis has been consistently and successfully employed as the definitive treatment for the periureteric fibrosis complicating Crohn's disease. The detection of ureteral obstruction as a consequence of regional enteritis is an indication for expedient operative treatment of the advanced inflammatory bowel disease and of its ureteral complication.
