Dr. Rahal's comments on the intrathecal use of gentamicin prompt us to report our serendipitous experience with intrathecal administration of gentamicin in dosage generally considered far in excess of that recommended. We recently saw a patient who developed an acute bacterial meningitis after a basilar skull fracture and "prophylactic" antibiotic therapy with ampicillin and kanamycin.A Gram-negative bacterial infection was presumed, and treatment with cephalothin and gentamicin was started. When the causative...References1. WATANAKUNAKORN C: Penicillin combined with gentamicin or streptomycin: synergism against enterococci. J Infect Dis 124:581-586, 1971 CrossrefMedlineGoogle Scholar2.
