To assess whether supplementing particulate bone substitute with an L-shaped soft-block bone substitute preserves horizontal volume stability during surgical flap closure in guided bone regeneration.
Created 20 peri-implant box-shaped defects across 10 porcine mandibles and evaluated two guided bone regeneration strategies sequentially per defect.
Compared particulate xenograft alone (control) against particulate xenograft combined with an L-shaped soft-block xenograft (test), both covered by a collagen membrane and secured with pins.
Acquired cone-beam computed tomography scans before and after flap suturing to evaluate horizontal, vertical, and angular dimensional changes.
Horizontal thickness reduction at the implant shoulder was significantly smaller in the test group (2.4 ± 9.2%) than in the control group (20.5 ± 23.3%; P = 0.014).
No statistically significant differences occurred between groups for horizontal thickness changes at 1 mm to 5 mm apical to the shoulder (P > 0.05).
Test group vertical thickness decreased by 28.0 ± 11.9% and 45° thickness decreased by 24.8 ± 10.2% following wound closure (P < 0.001).
