Prospective study reveals significant increases in anterior chamber angle, depth, and volume after laser peripheral iridotomy in angle closure glaucoma, indicating effective relief of crowding.
Background: Primary angle closure glaucoma has a characteristic feature of increased intraocular pressure resulting frommechanical obstruction of trabecular meshwork by either or by a synechial closed angle or apposition of the peripheral iris tothe trabecular meshwork. It has heavier risk of ocular morbidity compared to open angle glaucoma. With developingimaging devices for anterior segment of eye as Pentacam has better understanding of pathogenesis and of angle closure hasbeen attained. The present study was aimed to asses the changes in various parameters of anterior chamber parameters beforeand after peripheral iridotomy using laser in primary angle closure using pentacam.. Materials and Methods: The presentprospective study assessed 120 eyes from 90 subjects for subjects with Primary angle closure glaucoma that were managedat the Institute within the defined study period. These subjects were assessed for change in various parameters as ACVanteriorchamber volume, ACD-anterior chamber depth, ACA-anterior chamber angle, CCT-central corneal thickness)before and after Nd:YAG laser peripheral iridotomy (LPI). Results: In 120 eyes assessed, after Nd: YAG laser peripheraliridotomy, significant widening of anterior chamber angle was seen following Shaffer’s grading. The changes in ACV frombaseline to 1st week and baseline to first month was statistically significant. The mean ACA increase was seen from baselineto first week and to first month with p<0.05. For mean ACD increase from baseline to 1st week and 1st month wasstatistically significant. The shift in CCT values from baseline was statistically non-significant. Conclusion: The presentstudy confirms the results of the previous studies that reported increase in LPI, anterior chamber diameter, anterior chamberangle, and anterior chamber volume in study subjects with Primary angle closure glaucoma. Pentacam has been establishedas extremely useful tool for quantification of these changes.
