Careful preoperative evaluation, appropriate intraoperative techniques, and effective postoperative multimodal analgesia enable safe and effective ambulatory anesthesia in pediatric patients.
Careful preoperative evaluation, appropriate intraoperative techniques, and effective postoperative pain and nausea management are essential for safe same-day discharge in pediatric ambulatory anesthesia.
May support optimized protocols for pediatric ambulatory anesthesia; leaves open need for prospective trials.
A An na ah h t ta ar r K Ke e l li i m me e l le er r: : Pediatri; ayaktan cerrahi işlemler; anestezi A AB BS S T TR RA AC CT T A patient being discharged the same day of surgery is described as 'ambulatory surgery'; anesthesia administration for such surgeries is described as 'ambulatory anesthesia'.First presented in 1909 in Glasgow, ambulatory anesthesia and ambulatory surgery for the pediatric population, has become commonly preferable because of less disturbance for the patient and family, less occupation of hospital beds leading to less patient accumulation, less nosocomial infection risk and low cost.Nowadays it has been reported that more than 70% of all elective surgeries are ambulatory and that cases below the age of 15 has increased. With emerging surgery and anesthesia techniques, an important amount of patients will be able to be included in the coverage range of ambulatory surgery.Nevertheless, serious diseases accompanying the cause of surgery, long-term and/or traumatic surgeries and the need for postoperative follow-up can be considered drawbacks from ambulatory surgery. Therefore choosing the operation type is the primary question for administering ambulatory anesthesia.Preoperative evaluation, intraoperative anesthesia induction and maintenance and challenges postoperatively encountered, may differ from adults because of frequent upper airway infections in the pediatric population, vaccination programs, obesity and obstructive sleep apnea syndrome. For these reasons, in this review, operation suitability for ambulatory surgery, preoperative evaluation, intraoperative and postoperative topics which need attention in premedication administration and agents, postoperative pain control and patient care, early recovery and early discharge, are discussed.
