Key result
Low-dose triple or quadruple therapy lowers systolic blood pressure by ~13 mmHg versus placebo.
Why the study?
The study aimed to assess the efficacy and safety of low-dose combinations of 3 or 4 blood-pressure lowering drugs in people with untreated hypertension.
Do low-dose combinations of 3 or 4 blood-pressure lowering drugs reduce blood pressure in untreated individuals with hypertension?
Meta-Analysis (n=4,733)
Do low-dose combinations of 3 or 4 blood-pressure lowering drugs reduce blood pressure in untreated individuals with hypertension?
Mean Difference: -13.1 (95% CI -17.6–-8.6)
p-value: p=<0.001
Low-dose combinations of 3 or 4 antihypertensive drugs are highly effective and well-tolerated as initial therapy in untreated hypertension, significantly lowering blood pressure compared to placebo and monotherapy.
Supports low-dose 3- or 4-drug combinations as initial therapy in untreated hypertension; extends RCT evidence beyond dual therapy.

Rueter et al. (2026) conducted a meta-analysis in Untreated hypertension (n=4,733). Low-dose combinations of 3 or 4 blood-pressure lowering drugs vs. Placebo was evaluated on Systolic blood pressure (SBP) difference (MD -13.1 mmHg, 95% CI -17.6, -8.6, p=<0.001). Low-dose combinations of 3 or 4 antihypertensive drugs reduced systolic blood pressure by a mean of 13.1 mmHg compared to placebo in untreated patients.
