A case of oat cell carcinoma of the lung with hypokalemic alkalosis is presented. Plasma ACTH and plasma corticoids were within normal limits. Aldosterone secretion was increased but responsive to changes in sodium intake. Plasma renin activity levels were elevated and unresponsive to the changes in sodium diet. It is postulated that the electrolyte imbalance was produced by the increased aldosterone secretion maintained by the excessive amount of renin-like material produced by the tumor.
