Twenty patients have undergone a total of 28 revascularization procedures for symptomatic fibromuscular dysplasia of the internal carotid artery. A wide spectrum of preoperative complaints included transient cerebral ischemic symptoms and completed strokes. Only one patient sustained a transient postoperative neurologic deficit. All patients have had improvement or complete cessation of their preoperative complaints, and there have been no late postoperative strokes. The surgical techniques employed, including resection and reanastomosis and graduated internal dilation, have been successful in establishing widely patent and durable internal carotid artery lumens.
