Transapical aortic valve implantation using the J-Valve system was successfully performed in a patient with severe quadricuspid aortic valve regurgitation, resulting in normal valve function at 6 months.
Why the study?
Due to the rarity of quadricuspid aortic valve, treatment and prognosis of patients with aortic regurgitation remain challenging.

Transapical aortic valve implantation using the J-Valve system is a feasible and effective treatment for severe aortic regurgitation in patients with a rare quadricuspid aortic valve.
Successful TAVI with J-Valve feasible in severe QAV regurgitation; hypothesis-generating and should not yet change practice.
Background: Quadricuspid aortic valve (QAV) is a rare congenital heart defect usually accompanied with different hemodynamic abnormalities. Due to the rarity of QAV, treatment and prognosis of QAV patients with aortic regurgitation still remain challenging. We here present the first case of a patient with severe QAV regurgitation who underwent successful treatment and performed favorable prognosis with transapical aortic valve implantation (TAVI) using J-Valve system.Case presentation: A 62-year-old man experienced intermittent palpitation, shortness of breath and chest pain. Echocardiography revealed congenital QAV with massive aortic regurgitation and mild aortic stenosis, left ventricular enlargement. Aortic valve replacement was successfully performed with TAVI using J-Valve system. The postoperation and follow-up was uneventful. Conclusion: Transapical aortic valve implantation (TAVI) using J-Valve system has emerged as a new high success rate method for treatment of high-risk patients with simple non-calcified aortic valve insufficiency.

Luo et al. (2021) conducted a case report in Quadricuspid aortic valve with massive aortic regurgitation (n=1). Transapical aortic valve implantation (TAVI) using J-Valve system was evaluated on Procedural success and valve function. Transapical aortic valve implantation using the J-Valve system was successfully performed in a patient with severe quadricuspid aortic valve regurgitation, resulting in normal valve function at 6 months.
