Key result
Epidural dexmedetomidine significantly prolonged the duration of postoperative analgesia compared to clonidine (345.4 vs 303.4 minutes, p<0.001) in patients undergoing pelvis and lower limb orthopaedic surgeries.
Why the study?
Alpha 2 agonists are used as adjuvants in epidural analgesia for analgesia, anxiolysis, and sedation, motivating an evaluation of the postoperative analgesic effects of epidural dexmedetomidine versus clonidine with 0.125% levobupivacaine.

Epidural dexmedetomidine provides prolonged postoperative analgesia compared to clonidine in patients undergoing lower limb and pelvic orthopaedic surgeries.
May support epidural alpha-2 agonists with levobupivacaine; leaves open comparative efficacy and optimal use in postoperative analgesia.
Background: Alpha 2 agonists are being used as adjuvants in epidural analgesia as they lead to analgesia, anxiolysis and sedation. Aim: This study aims to evaluate the post operative analgesic effects of epidural dexmedetomidine and clonidine with 0.125%levobupivacaine Methods: A comparative study was done to assess the efficacy and clinical profile of two 2 adrenergic agonistsclonidine and dexmedetomidine administered epidurally in combination with intrathecal 0.125%levobupivacaine. The study was conducted for 1 year. Sixty adult patients ASA-PS Class I and II undergoing pelvis and lower limb orthopaedic surgery were randomly assigned into two groups, to receive either epidural dexmedetomidine (1g/kg) or clonidine (2 g/kg) with 0.125% isobaric levobupivacaine. Results: Dexmedetomidine provided a prolonged analgesia, as evidenced by the distribution of VAS scores and requirement rescue analgesic among two groups.

Dr Amutha Rani R (2022) conducted an RCT in Postoperative pain following pelvis and lower limb orthopaedic surgeries (n=60). Dexmedetomidine vs. Clonidine 2 μg/kg epidurally with 0.125% levobupivacaine was evaluated on Duration of analgesia (minutes) (p=<0.001). Epidural dexmedetomidine significantly prolonged the duration of postoperative analgesia compared to clonidine (345.4 vs 303.4 minutes, p<0.001) in patients undergoing pelvis and lower limb orthopaedic surgeries.
