Summary We report a case of delayed‐onset severe lower back pain following two epidural blood patch procedures for post‐dural puncture headache following caesarean section with spinal anaesthesia. An urgent magnetic resonance imaging confirmed the diagnosis of a spinal subdural haematoma. Surgical decompression was not required and the patient improved with conservative management and analgesia. Though back pain following an epidural blood patch procedure is relatively common, this case highlights the risk and management of this rare complication.
