Hospital-acquired disability (HR 1.857) and being underweight (HR 1.941) independently predicted increased all-cause mortality after discharge in older patients with acute decompensated heart failure.
Why the study?
To investigate the effect of hospital-acquired disability on all-cause mortality after discharge according to body mass index in older patients with acute decompensated heart failure.
Does hospital-acquired disability increase all-cause mortality in older patients with acute decompensated heart failure across different BMI categories?
Cohort (n=408)
No
Does hospital-acquired disability increase all-cause mortality in older patients with acute decompensated heart failure across different BMI categories?
Hospital-acquired disability significantly increases the risk of post-discharge all-cause mortality in older patients with acute decompensated heart failure, especially in those who are underweight.
HAD signals elevated post-discharge mortality risk in older ADHF patients; leaves open whether preventing HAD improves survival across BMI categories.
OBJECTIVES: To investigate the effect of hospital-acquired disability (HAD) on all-cause mortality after discharge according to the body mass index (BMI) in older patients with acute decompensated heart failure. METHODS: ). HAD was defined as a decrease of at least five points at discharge compared to before hospitalization according to the Barthel Index. CONCLUSIONS: BMI and HAD independently predicted all-cause mortality after discharge in older patients with acute decompensated heart failure. Furthermore, HAD was significantly associated with higher all-cause mortality after discharge, especially in the underweight group.

Sakuyama et al. (2022) conducted a cohort in Acute decompensated heart failure (n=408). Hospital-acquired disability (HAD) and body mass index (BMI) vs. No hospital-acquired disability and normal BMI was evaluated on All-cause mortality after hospital discharge (HR 1.857, 95% CI 1.062-3.250, p=0.030). Hospital-acquired disability (HR 1.857) and being underweight (HR 1.941) independently predicted increased all-cause mortality after discharge in older patients with acute decompensated heart failure.
