SUMMARY Direct measurement of the pressures which develop within the bladder during voiding offers a new approach to the diagnosis of “occult” lower urinary tract obstruction. Insight into the pathological physiology of some hitherto confusing syndromes such as hydro‐ureter is offered by this technique. Experimental studies utilising the information gained in this way offer an explanation for upper urinary tract infection and bacteremia from lower urinary tract infection.
