HybridnĂ ablace (hybrid ablation, HABL) fibrilace sĂnĂ jako kombinace endoskopickĂŠ, minimĂĄlnÄ invazivnĂ, epikardiĂĄlnĂ ablace pĹi zavĹenĂŠm hrudnĂku spolu s pĹesnostĂ endokardiĂĄlnĂ metody vedenĂŠ systĂŠmem CARTO byla zavedena ve snaze pĹekonat limitace souÄasnĂ˝ch moĹžnostĂ lĂŠÄby pacientĹŻ s perzistujĂcĂ fibrilacĂ sĂnĂ (persistent atrial firbrillation, PSAF) a dlouhodobÄ perzistujĂcĂ fibrilacĂ sĂnĂ (long-standing persistent atrial fibrillation, LSPAF). Smyslem toho monocentrickĂŠho, prospektivnĂho klinickĂŠho registru bylo zhodnotit bezpeÄnost a proveditelnost i ĂşÄinnost HABL u pacientĹŻ s PSAF a LSPAF jeden rok po vĂ˝konu. V obdobĂ od Äervence 2009 do prosince 2014 byla HABL provedena celkem u devadesĂĄti (n = 90) pacientĹŻ (PSAF, n = 39 a LSPAF, n = 51). PrĹŻmÄrnĂĄ dĂŠlka incidence fibrilace sĂnĂ byla 4,5 ± 3,7 roku. Ĺ est mÄsĂcĹŻ po ablaci bylo 78 % pacientĹŻ v sinusovĂŠm rytmu. DvanĂĄct mÄsĂcĹŻ po vĂ˝konu bylo v sinusovĂŠm rytmu 86 % pacientĹŻ a 62,3 % v sinusovĂŠm rytmu bez podĂĄvĂĄnĂ antiarytmik skupin I/III. Na zĂĄkladÄ tÄchto vĂ˝sledkĹŻ lze usuzovat, Ĺže u pacientĹŻ s perzistujĂcĂ a dlouhodobÄ perzistujĂcĂ fibrilacĂ sĂnĂ je tĹeba uvaĹžovat o kombinaci epikardiĂĄlnĂ a endokardiĂĄlnĂ radiofrekvenÄnĂ ablace, protoĹže se jednĂĄ a bezpeÄnou a ĂşÄinnou metodu obnovy sinusovĂŠho rytmu.
