Cystic artery pseudoaneurysm is a rare complication of acute cholecystitis. Its presentation has been related to inflammatory damage to the adventitia or traumatic origin after invasive procedures. Angiography is both a diagnostic and therapeutic modality that allows its management through interventional radiology. We present the case of a 60-year-old man with acute cholecystitis and subsequent deterioration of his clinical condition, associated with hemobilia and hemodynamic compromise, in whom the diagnosis of ruptured pseudoaneurysm of the cystic artery was established, which was managed urgently through transarterial embolization and subsequent laparoscopic cholecystectomy.
