Transesophageal echocardiography successfully diagnosed an inverted left atrial appendage following mitral valve replacement, allowing for conservative management without unnecessary intervention.
Why the study?
Inverted left atrial appendage is a rare occurrence after cardiac surgery that can be misinterpreted as a left atrial thrombus, mass, or vegetation.
Case Report (n=1)
TEE is crucial for diagnosing inverted left atrial appendage after cardiac surgery, preventing misdiagnosis as a thrombus or mass and avoiding unnecessary interventions.
TEE differentiation of inverted LAA may avoid unnecessary interventions post-MVR; leaves open broader validation in surgical cohorts.
An inverted left atrial appendage (ILAA) is a rare occurrence after cardiac surgery and can be misinterpreted as a left atrial thrombus, mass, or vegetation. Transesophageal echocardiography (TEE) used post-operatively is useful in detailed visualization of the left atrium and any abnormality within it. We present a case in which a patient developed an ILAA post mitral valve replacement (MVR) and was diagnosed using TEE. We want to emphasize the importance of TEE post-operatively and that differentials for a left atrial mass must include ILAA, especially after cardiac surgery. This can help in preventing unnecessary anticoagulation and the need for surgical revision. • An inverted left atrial appendage should be considered a high-priority differential diagnosis in patients presenting with a left atrial appendage mass following cardiac surgery. • Transesophageal echocardiography (TEE) is the preferred diagnostic modality for suspected cases of inverted left atrial appendage. • Once diagnosed, an inverted left atrial appendage can often be managed conservatively without the need for intervention.

Bahar et al. (2024) conducted a case report in Inverted left atrial appendage post mitral valve replacement (n=1). Transesophageal echocardiography (TEE) was evaluated on Diagnosis of inverted left atrial appendage. Transesophageal echocardiography successfully diagnosed an inverted left atrial appendage following mitral valve replacement, allowing for conservative management without unnecessary intervention.
