Implication

Highlights periprocedural stroke risk; extends single-center feasibility data but leaves open multicenter validation.

Structured PICO

P
Population
110 nonconsecutive patients with extracranial carotid artery occlusive disease (symptomatic with ≥ 70% stenoses or ulcerative lesions, or asymptomatic with ≥ 75% stenoses), mean age 72 years, 72% male, 72% asymptomatic.
I
Intervention
Intravascular stenting (mostly Palmaz balloon-expandable stents) preceded by standard balloon dilation, with postprocedural aspirin only.
O
Outcome
Clinical success at 30 days (defined as no technical failure, death, endarterectomy, stroke, or occlusion)composite

Carotid stenting is technically feasible with good midterm patency, but the high rate of periprocedural neurological events highlights the need for improved techniques and more aggressive antiplatelet regimens.

Limitations

  • High incidence of neurological sequelae
