ST segment alternans can occur during elective balloon angioplasty, representing a continuum of ST segment changes secondary to transmural myocardial ischemia.
ST-segment alternans during elective LAD angioplasty may signal transmural ischemia; single case leaves open its prognostic value and monitoring role.
ST segment alternans has been described in experimental coronary artery occlusion and in patients with variant angina. It is also seen during coronary angioplasty and may represent a continuum of the ST segment changes secondary to transmural myocardial ischemia. Its presence has been associated with ventricular arrhythmias and its relative infrequency attributed to the coexistent use of calcium channel blockers in patients undergoing elective angioplasty. We describe a patient with this phenomenon during balloon angioplasty of the left anterior descending coronary artery with a review of the literature. (J Interven Cardiol 1993; 6:231–234)
