The concentrations of diphellylhydantoin (DPH) alld phenoharhital have been measured in plasma, lumbar cerebrospinal fluid (CSF), and samples of temporal lobe in 12 epileptic patients undergoing temporal lobectomy. The brain:plasma ratio of DPH was 0.75 ± 0.19:1 and the CSF: plasma ratio 0.12 ± 0.04:1. For phenobarhital, the mean hrain:plasma ratio was 0.59 ± 0.21:1 and the CSF: plasma ratio 0.46 ± 0.12:1. The significant correlation (p < 0.01) between brain and plasma drug concentrations supports the value of monitoring plasma concentrations of anticonvulsant drugs in patients with epilepsy.
