Factor XIII administration can be an effective hemostatic strategy for post-extraction bleeding in patients with chronic DIC and acquired factor XIII deficiency associated with thoracic aortic aneurysm.
May support factor XIII in refractory post-extraction bleeding with acquired deficiency; hypothesis-generating and requires prospective validation.
Chronic DIC associated with thoracic aortic aneurysm may lead to acquired factor XIII deficiency due to factor XIII consumption. This report describes a case of post-extraction bleeding due to chronic DIC and acquired factor XIII deficiency, in which the administration of factor XIII was effective for hemostasis.A 79-year-old man was referred to our hospital due to gingival swelling of the right maxillary first molar. The patient had a medical history of a thoracic aortic aneurysm.Tooth extraction was performed, and hemostasis was achieved the next day. From the sixth hospital day, the patient began to bleed persistently and wound healing failure became evident.Blood tests showed an increase in fibrinolytic markers and a decrease in factor XIII.As for treatment, hemostasis was achieved by systemic administration of factor XIII concentrate and wound sealing with polyglycolic acid sheets and fibrin glue containing factor XIII.When dealing with postextraction bleeding in patients with chronic DIC with thoracic aortic aneurysm, it is important to consider the possibility of factor XIII deficiency and to take measures appropriate to the disease condition.

Hyodo et al. (2024) conducted a case report in Post-extraction bleeding due to chronic DIC and acquired factor XIII deficiency (n=1). Factor XIII concentrate was evaluated on Hemostasis and wound healing.
