Key result
Serum chemerin, adiponectin, and point shear wave elastography accurately predicted simple steatosis, NASH, and cirrhosis in NAFLD patients, with AUCs ranging from 0.928 to 1.00.
Why the study?
Liver biopsy is the invasive gold standard for diagnosing NAFLD, making noninvasive diagnostic markers such as chemerin and adiponectin desirable.
Can serum chemerin, adiponectin, and point shear wave elastography be used as non-invasive diagnostic markers for NAFLD?
Case-Control (n=90)
Can serum chemerin, adiponectin, and point shear wave elastography be used as non-invasive diagnostic markers for NAFLD?
Serum chemerin, adiponectin, and point shear wave elastography demonstrate high diagnostic accuracy as non-invasive markers for NAFLD, NASH, and cirrhosis.
Hypothesis-generating for noninvasive NAFLD staging; prospective validation required before clinical adoption.
Objective: To study the levels of serum adiponectin and chemerin in patients with nonalcoholic fatty liver disease(NAFLD) and their diagnostic role. Background: NAFLD is the most common cause of chronic liver disease, which is a major cause of morbidity and mortality. Liver biopsy remains the gold standard for its diagnosis, but it is an invasive procedure. chemerin and adiponectin can be possible noninvasive diagnostic markers. Patients and methods: This prospective study was conducted from March 2020 to October 2021 on 90 subjects including 60 patients with NAFLD and 30 age and gender-matched healthy individuals as control group. All were subjected to history taking, complete medical examination, thorough laboratory and radiological investigations. Serum chemerin and adiponectin were measured by enzyme linked immunosorbent assay (ELISA) technique and liver stiffness(LS) was measured by point shear waveelastography (pSWE). Results: High statistically significant elevation of serum chemerin levels and pSWE measurements in NAFLD patients than control. High statistically significant decrease of serum adiponectin levels in NAFLD patients than control. Serum chemerin levels can predict patients with simple steatosis(at cutoff point more than 280 ng , nonalcoholic steatohepatitis (NASH)( at cutoff point more than 698 ng )and cirrhosis (at cutoff point more than 997.5 ng ) and AUC was (0.988, 0.951and 1) respectively, while, adiponectin levels can predict simple steatosis (at cutoff point less than 9 ug ), NASH (at cutoff point less than 6.95 ug) and cirrhosis( at cutoff point less than 5.40 ug) and AUC was ( 0.996, 0.999 and 0.984) respectively. LS measurements by pSWE can predict patients with simple steatosis ( at cutoff point more than 6.10kpa), NASH (at cutoff point more than 6.85kpa ) and cirrhosis (at cutoff point more than 9.85kpa ) and AUC was (0.928, 0.984 and 1.00) respectively. Conclusion: Serum chemerin, adiponectin and point shear wave elastography, can be used as non-invasive parameters for diagnosis of NAFLD.

Seleem et al. (2023) conducted a case-control in Nonalcoholic fatty liver disease (NAFLD) (n=90). Serum chemerin, adiponectin, and point shear wave elastography vs. Healthy controls was evaluated on Prediction of simple steatosis, NASH, and cirrhosis. Serum chemerin, adiponectin, and point shear wave elastography accurately predicted simple steatosis, NASH, and cirrhosis in NAFLD patients, with AUCs ranging from 0.928 to 1.00.
