Key result
Dynamic compression plating resulted in a higher rate of good to excellent functional outcomes (90% vs 61%) and earlier union compared to interlocking intramedullary nailing for humeral fractures.
Why the study?
Does Dynamic Compression Plate (DCP) improve functional outcomes and union time compared to Intra Medullary Interlocking Nail (IMILN) in patients with acute humeral shaft fractures?

Does Dynamic Compression Plate (DCP) improve functional outcomes and union time compared to Intra Medullary Interlocking Nail (IMILN) in patients with acute humeral shaft fractures?
Absolute Event Rate: 90% vs 61.1%
Dynamic compression plating shows better functional results and earlier union time compared to interlocking intramedullary nailing for acute humeral shaft fractures.
DCP may offer better functional results than IMILN; leaves open superiority in larger randomized trials before changing practice.
Background: Fracture of the diaphysis of humerus and its complications are a major cause of morbidity in trauma patients. Fracture of the humeral shaft account for 20% of humeral fractures & about 3% of all fractures. There is a debate between the choices of operation in humeral shaft fractures. Methods: A comparative study of management of acute humeral shaft fractures treated by Dynamic Compression Plate (DCP) and Intra Medullary Interlocking Nail (IMILN) fixation over a period of one half years. 18 patients of IMILN and 20 patients of DCP were included after considering inclusion and exclusion criteria. Functional scoring criteria were used for postoperative assessment & the average follow up period was one year. Results: A high rate of excellent & good results & a tendency for early union was seen with the plating group than nailing group. Conclusions: Plating shows better results than nailing.

Mamood et al. (2017) conducted a cohort in Acute humeral shaft fractures (n=38). Dynamic Compression Plating (DCP) vs. Interlocking Intramedullary Nailing (IMILN) was evaluated on Good to excellent functional results (Rodriguez-Merchan criteria). Dynamic compression plating resulted in a higher rate of good to excellent functional outcomes (90% vs 61%) and earlier union compared to interlocking intramedullary nailing for humeral fractures.
